We are changing our Mid-Year Service Review of Personal Plans. As per the new policy, the mid-year reviews are no longer required at 4 and 8 months and will now happen within 6 months of the most recently completed Personal Plan.
 
Along with these changes, we have updated the Mid-Year Service Review Form.  From this point forward, please be sure to use the updated form found on CH Connect.
 
We also recognize that as the Personal Plan cycle for each person is highly individualized, there may be questions about how to apply this new policy timeline, especially for those who may already be in the middle of an active annual Personal Plan cycle.
 
We will be held to our new standard frequency outlined in policy.  That means that there needs to be a review and/or Personal Plan update every 6 months.  With that in mind, here are a few recommendations in terms of how we transition to this new frequency:
  • If a person has just completed an annual Personal Plan, you may apply the new 6-month review moving forward.
  • If a person has just completed an 8-month review or is due or overdue for an annual Personal Plan, you may update the Personal Plan, and apply the new 6-month review moving forward.
  • If a person has just completed a 4-month review, you may do one of the following:
    • Complete an 8-month review as you would before, and then start the new 6-month review after the next annual Personal Plan; OR,
    • Update the Personal Plan early (within 6 months of the 4-month review) and apply the new 6-month review moving forward.
