We are working towards a future where people have greater choice and control over their services and Onward is helping us do that. One of the many activities underway is including F2023 Passport planning into the Personal Planning process. This includes completing the “F2023 Passport Planning Tool” for people who have agreed to have Christian Horizons manage their Passport Funds in F2023. This planning tool replaces the previous Passport Budgeting tool and allows people to plan their Passport Services for the next 12 months until their next Annual Personal Plan.
What you need to know
On Friday December 2, 2022, a new TAMS Item called “Personal Plan Review – F2023 Passport Planning Tool Completed” will be set up to track the completion of the “F2023 Passport Planning Tool.” This TAMS item will be activated for each person where Christian Horizons manages their Passport funding. The expectation is that the “F2023 Passport Planning Tool” will be incorporated into the Personal Plan and completed for each person by March 31, 2023. An excel or PDF copy of the “F2023 Passport Planning Tool” should be attached to the “Personal Plan Review – F2023 Passport Planning Tool” TAMS item.
The due date for the TAMS item has been set as December 17, 2022. This date was chosen because many personal plans have already been completed for the current fiscal year. If the Personal Plan is not scheduled to be completed until January-March 2023, the TAMS will remain as incomplete until the Personal Plan and corresponding Passport Planning Tool are completed.
For more information on the integration of Passport into the Personal Planning Process, please register for “Passport & Planning Information Session” in the LMS. This session is an hour long and provides more details on why and how we are integrating Passport into the Personal Plan. This session is available to all employees who are part of the Personal Planning process.

FAQ About the “F2023 Passport Planning Tool
Who should complete the “F2023 Passport Planning Tool”
The “F2023 Passport Planning Tool” should be completed along with the person’s Annual Personal Plan where Christian Horizons manages their Passport. The tool can be completed by the Independent Personal Plan Facilitator, Program Manager, or DSP completing the Personal Plan. If the person had Passport managed by Christian Horizons in F2022, the Program Manager should be involved in providing details and support from the F2022 Passport Budget.
What if the Annual Personal Plan was already completed without the “F2023 Passport Planning Tool”?
The Program Manager should review the Personal Plan to see if Passport was included in the Personal Plan and should complete the “F2023 Passport Planning Tool” based on the plan discussed. If Passport was not included with the Personal Plan than a separate discussion should be led by the Program Manager with the person receiving Passport Supports (and their advocates if required), to document the plan for their Passport funds. This could be done at the 4 or 8 month Check-in Service Review but should be done no later than March 2023.

Each time you are starting a new Passport Planning tool, please download a clean copy from the available locations as changes to this form are periodically made.

Where should the “F2023 Passport Planning Tool” be saved?
The “F2023 Passport Planning Tool” should be attached to TAMS item “Personal Plan Review – F2023 Passport Planning Tool”. It can be attached as an Excel workbook or PDF. A copy can also be saved on the V:Drive V:Agreements – PassportPurchasedServices.
Is the “F2023 Passport Planning Tool” required for people who manage their own Passport Funds?
If people manage their own Passport funds, the tool is not required. The tool can still be used as part of the Personal Planning process if desired. If the tool is used as part of their Personal Plan, it can be uploaded along with the other Personal Plan documents into the ODB.
