There are important updates to the Personal Plan process that are effective May 1, 2024. Please refer to a memo that was posted in “On the Horizon” on April 22, 2024 HERE: Personal Plan and Passport Update – info.chconnect.org.
On May 3, 2024 we hosted a webinar for Program Managers and any employees involved in Personal Planning to attend. On May 31 we hosted a follow up webinar for people who use services, families, and Karis employees about these updates.
During these webinars, people had the opportunity to submit questions. We answered as many questions as we could and now have a published list of FAQ from each webinar.
