As an update to our On the Horizon article dated December 1, 2022 – “New TAMS (Tracking and Monitoring System) Item: F2023 Passport Planning Tool” – we are providing more specific information on what roles are responsible for integrating Passport into the Personal Plan. On CH Connect, you will find two “F2023 Passport and Personal Planning Process” flow charts that provide specific details on who is responsible and how the Passport plan is documented in the “F2023 Passport Planning Tool.” These flowcharts also provide details on how the process is changed if an Independent Facilitated Personal Plan is completed for the person in the current fiscal year.
The two flow charts available are:
– “F2023 Passport and Personal Planning Process where Christian Horizons Manages a Person’s Passport Funds” (VPN Required)
– “F2023 Passport and Personal Planning Process where Christian Horizons does not manage a Person’s Passport Funds” (VPN Required)
Coloured printed copies of the flow charts have been sent to program locations to make this resource accessible for all staff.
For more information on the integration of Passport into the Personal Planning Process, please register for “Passport & Planning Information Session” in the LMS. This session is an hour long and provides more details on why and how we are integrating Passport into the Personal Plan. This session is available to all employees who are part of the Personal Planning process.
