We need your help to track COVID-19 vaccinations!
Please let us know if you have had one or two vaccinations by submitting your data via one of the following sites. It is a very simple and easy to use tool. You can submit your data at a service location using any Christian Horizons device that is connected to a Christian Horizons network, or through the VPN.
