The HR Admin team, in collaboration with Recruitment, the Employee Engagement Representatives, and Legal have revised the P-09-11 form, HRIS Employee Change Form, to simplify and clarify its use.

The updated form is now available on CH Connect.

What’s Changed?

  • Removed duplicate headers in each section for improved readability.
  • Removed fields requesting information not necessary for the change.
  • Added a statement in the first section requiring the employee to initial the change.

When to Use a P-09-11

P-09-11s should only be used for changes that do not require a job competition such as:

  • Adding or removing a location with no commitment of hours for occasional emergency coverage
  • Adding or removing a position with no commitment of hours for occasional emergency coverage. This is only permitted in the following scenarios:
    • Part-time/full-time DSP adding a Night Support job.
    • Part-time/full-time Night Support employees adding a DSP job.
    • Casual DSP adding a casual night sleep job.

When Not to Use a P-09-11

  • When the employee is changing employment type (e.g.: full-time to part-time, part-time to casual, casual to full-time)
  • When a full-time or part-time employee is changing their primary location of work, this requires a job competition to move the hours to the new location.
  • For new hires
  • For end of employment
